Discreet Signs in Ordinary Psychoses

What should we look for?

What is discreet? Inconspicuous, subtle, unnoticeable…Ok, so I have to look for those small details, or maybe divine details?

Signs, what are signs? They can be from cyphers to symbols to secret code/language to emblems.

Can this be a tool of clinical orientation in the mental health clinic where I work in the most diverse ethnic neighborhood of the entire USA, where services are provided in more than 20 languages? ,

So diverse that we have to consider the particularities of each case in reference to their culture.

My wager is that this compass is an extremely valuable tool that can orient our practice and the direction of the treatment. While the diagnoses of the DSM give us global categories by checking off a list of symptoms, according to the time of onset, duration and severity, in a quantitative and statistical dimension, our Lacanian concepts may help us find our way in the modern towers of Babel.

Let’s take two basic categories of Depression and Anxiety. Are all depression the same? Are all anxieties the same? What are the discreet signs we should be looking for?

A symptom, or rather, a sensation commonly described by patients is  “blankness”, the feeling that their mind went blank, that their head is empty. This can produce a lot of anxiety and depression, fear of losing one’s mind, of having dementia or Alzheimer.

I think we can connect this to what Jacques-Alain Miller developed in the Conversation of Arcachon as an unplugging (débranchement). These episodes can have different time variations, last for a few minutes or longer but they are always lived as something strange, and also something the subject is afraid of and ashamed to talk about. In the Latino population of this mental health clinic it could be represented as a variation of what Almodovar described in that extraordinary film “Women on the Verge of a Nervous Breakdown”.

This “unplugging” is it a symptom or a sign? Even though the subject situates it in the mind, the head, can we speak of it as a body event? Is it an invasion of jouissance in the body or better still an emptying out of jouissance. My proposal is that it is the latter, as if the wall of language and images were like a sieve, incapable of containing jouissance as it leaks through its porous wall; therefore, the intense effort of the subject to hold onto something.

In Ordinary Psychosis Revisited, Jacques-Alain Miller situates this as a Subjective Externality: “its an experience in the ordinary psychotic of void, of emptiness and vagueness”. He says that we can encounter this in various cases of neurosis but in ordinary psychosis what we are looking for is “…a clue of a non-dialectisible quality of the void or of the vagueness. There is a special fixity of this element.” (Psychoanalytical Notebooks 19, p. 157)

The key words here are non-dialectisible and fixity. These are some discreet signs we may look for.

So in this non-dialectisible logic what is operative? Not the meaning which in itself can be delusional. But we need to invent, in each case, an apparatus, a prosthesis, that would allow the subject to reconnect, and that would stop or diminish the leakage, an artificial bond to re-appropriate his/her mind to itself.

Linda complains of symptoms of “depression” and “anxiety”. She wakes up crying every day, she doesn’t know why, she doesn’t ask herself why either. She just says she doesn’t know what is happening to her. This happening has been gone on since 9/11, when the Twin Towers collapsed in New York. Why? She doesn’t know, she saw it on T.V., she was not near that place, she doesn’t know anyone who died or suffered directly from the terrorist attack. Since then her world collapsed. She has no delusions, no hallucinations, only multiple physical illnesses that require medical services. Each session is like a new session, nothing is built from previous sessions. When she comes, she cries, complains, describes her suffering, calms down and leaves, week after week for years.

What function do I occupy? A safe space, where she can come and complain without being judged or rejected. The fixity and the repetition are the salient factors in this case. But the question remains how to make the surface less porous?

Till a man has accomplished twelve books of poetry, the same is not taken for want of poetry but is forced away. No man is taken till a black hole is hollowed in the world to the depth of his two oxters and he put into it to gaze from it with his lonely head and nothing to him but his shield and a stick of hazel. Then must nine warriors fly their spears at him, one with the other and together. If he be spear‐holed past his shield, or spear‐killed, he is not taken for want of shield‐skill. No man is taken till he is run by warriors through the woods of Erin with his hair bunched‐loose about him for bough‐tangle and briar‐twitch. Should branches disturb his hair or pull it forth like sheep‐wool on a hawthorn, he is not taken but is caught and gashed. Weapon‐quivering hand or twig‐crackling foot at full run, neither is taken. Neck‐high sticks he must pass by vaulting, knee‐high sticks by stooping. With the eyelids to him stitched to the fringe of his eye‐bags, he must be run by Finn’s people through the bogs and the marsh‐swamps of Erin with two odorous prickle‐backed hogs ham‐tied and asleep in the seat of his hempen drawers. If he sink beneath a peat‐swamp or lose a hog, he is not accepted of Finn’s people. For five days he must sit on the brow of a cold hill with twelve‐pointed stag‐antlers hidden in his seat, without food or music or chessmen. If he cry out or eat grass‐stalks or desist from the constant recital of sweet poetry and melodious Irish, he is not taken but is wounded. When pursued by a host, he must stick a spear in the world and hide behind it and vanish in its narrow shelter or he is not taken for want of sorcery. Likewise he must hide beneath a twig, or behind a dried leaf, or under a red stone, or vanish at full speed into the seat of his hempen drawers without changing his course or abating his pace or angering the men of Erin. Two young fosterlings he must carry under the armpits to his jacket through the whole of Erin, and six arm‐bearing warriors in his seat together. If he be delivered of a warrior or a blue spear, he is not taken. One hundred head of cattle he must accommodate with wisdom about his person when walking all Erin, the half about his armpits and the half about his trews, his mouth never halting from the discoursing of sweet poetry. One thousand rams he must sequester about his trunks with no offence to the men of Erin, or he is unknown to Finn. He must swiftly milk a fat cow and carry milk‐pail and cow for twenty years in the seat of his drawers. When pursued in a chariot by the men of Erin he must dismount, place horse and chariot in the slack of his seat and hide behind his spear, the same being stuck upright in Erin. Unless he accomplishes these feats, he is not wanted of Finn. But if he do them all and be skilful, he is of Finn’s people.

« Mon second cas n’aurait certainement pas été classé, sans analyse, comme paranoïa persecutoria, mais je dus considérer le jeune homme comme un candidat à cette issue morbide. Il existait chez lui une ambivalence dans le rapport au père d’une envergure tout-à-fait extraordinaire. Il était d’un côté le rebelle qui se soit manifestement et en tous points développé à l’écart des désirs et des idéaux du père ; d’un autre côté, à un niveau plus profond il était toujours le plus soumis des fils, celui qui après la mort du père se frustrait de la jouissance de la femme dans une tendre conscience de culpabilité. Ses relations réelles aux hommes se tenaient manifestement sous le signe de la méfiance ; grâce à son puissant intellect il savait rationaliser cette position et s’entendait à arranger les choses de telle sorte que ses connaissances et amis le trompent et l’exploitent. Ce que j’ai appris de nouveau avec lui c’est que les classiques idées de persécutions peuvent exister sans qu’on leur accorde de crédit ni de valeur. incidemment elles passaient comme des éclairs dans son analyse mais il ne leur accordait aucune importance et les tournait régulièrement en ridicule. Il peut en aller de même dans beaucoup de cas de paranoïa, et quand une telle affection éclate nous tenons peut-être les idées délirantes exprimées pour des productions nouvelles alors qu’elle peuvent avoir été présentes depuis longtemps. 
 
Une idée importante me parait être qu’un facteur qualitatif, à savoir la présence de certaines formations névrotiques, importe moins dans la pratique que le facteur quantitatif, à savoir le degré d’attention, plus exactement la mesure d’investissement que ces formations peuvent attirer

 

La discussion de notre premier cas, cas de paranoïa de jalousie, nous avait pareillement incité à mettre en valeur le facteur quantitatif, en nous montrant qu’en l’occurence l’anormalité consistait essentiellement dans le surinvestissement des interprétations de l’inconscient étranger. Par l’analyse de l’hystérie nous connaissons depuis longtemps un fait analogue. Les fantasmes pathogènes, les rejetons des motions pulsionnelles refoulées, sont longtemps tolérés à côté de la vie psychique normale et n’ont pas d’effet pathogène jusqu’à ce qu’ils reçoivent un surinvestissement dû à un revirement de l’économie libidinale ; ce n’est qu’alors qu’éclate le conflit qui conduit à la formation du symptôme.

 

Ainsi le progrès de notre connaissance nous pousse toujours de plus en plus à mettre au premier plan le point de vue économique  »
